Liability Waiver

I understand that speed skating and training for competition are potentially hazardous activities which include the risks of injury. I understand that I am solely responsible for the registering skater's health and medical condition, and that it is my responsibility to determine, along with a physician, the skater's physical and mental fitness to undertake a strenuous training program. I assume all risks, including injury and death, associated with or arising out of the registering skater's participation in this training program (including COVID-19), and I hereby waive and release the coaches and Board of Directors of UCB from all claims and liabilities of any kind or description arising out of my child's participation in this program.
